It has been a privilege to serve your medical needs over the past 16+ years through OnSite for Seniors. What began in 2008 as a small, faith-based 501(c)(3) house-call program has grown alongside the changing needs of seniors and their families in our community.
 

As part of a shift in focus, OnSite for Seniors discontinued providing specialized medical care in the community on August 30, 2024. 
 

Medical Records Requests

To transfer your medical records:

1. Complete the Medical Records Authorization Form (download required).

2. Send the signed form to:

Please note:

• Fees may apply depending on the size and processing of your request.

• Records will be available for 10 years (through August 30, 2034) or as required by Idaho law.
